Job Description

Care Professional (Care Pro) Care Professionals provide non-medical home care services to individuals in their own homes and communities who need assistance due to aging, illness, disability, or other conditions. Services may include assistance with activities of daily living (ADLs), household tasks, companionship, and support with maintaining a safe and comfortable home environment. All services are provided in a caring, respectful manner in accordance with the client's plan of care, agency policies, and applicable state regulations.
Service Area:
South Charlotte and surrounding areas including Matthews, Ballantyne, and nearby communities (ZIP codes 28105, 28270, 28277, 28226, 28210, 28134). Care Professionals are assigned to clients within our service area; travel between client homes is required. Reporting Relationship Reports to the Supervisor (RN Supervisor / Service Supervisor) Key ResponsibilitiesPersonal Care and Daily Living Support Assist clients with activities of daily living, including: Bathing Mouth care Dressing Hair care Feeding Toileting Medication reminders Skin care Exercise and mobility support Safety and Supervision Ensure client safety by monitoring the home environment Observe and report unsafe conditions to the supervisor Follow all safety procedures and care guidelines Household Support (IADLs) Assist with light housekeeping (kitchen, bathroom, laundry) Make and change beds Assist with meal planning and preparation Help maintain an organized and safe home Companionship and Emotional Support Provide companionship and social interaction Engage in conversation and meaningful activities Offer emotional reassurance and encouragement Support activities that stimulate the mind Client Support and Daily Needs Assist with errands and shopping (as outlined in the care plan) Escort clients to appointments, outings, or activities Support adherence to dietary plans as directed in the care plan Assist clients with communication (e.g., writing, light coordination tasks) Care Plan and Team Participation Follow the written plan of care at all times Participate as a member of the care team Provide input and observations to the supervisor Monitoring and Reporting Observe and report: Physical changes Behavioral or cognitive changes Changes in living conditions Report concerns promptly to the supervisor Document daily activities, care provided, and hours worked accurately Professional Responsibilities Attend orientation, training, and in-service sessions Follow all agency policies and procedures Maintain professional and respectful relationships with clients, families, and team members Communicate effectively with supervisors and coworkers Perform other duties as assigned Qualifications Valid driver's license and reliable transportation Ability to pass required background and registry checks Ability to follow written care plans and instructions Strong interpersonal skills Ability to communicate effectively in English to follow care plans, ensure client safety, and report updates to the supervisor Dependable and able to work independently Work Environment Services are provided in client homes and may involve standing, walking, bending, lifting, and assisting clients with mobility. Important Note This position provides non-medical care only . All duties are performed in accordance with agency policies, the client's plan of care, and applicable state regulations.
Pay:
$15.00 - $18.00 per hour
