As a Caregiver DSP, you will be an integral part of creating a safe, respectful, and empowering atmosphere for residents, promoting independence and quality of life through attentive caregiving and personalized support. Position Summary The Direct Service Provider is responsible for providing direct supervision, support, and assistance to residents in the supervised living home and for helping maintain a safe, structured, respectful, and recovery-oriented environment. The DSP assists residents with daily routines, community integration, behavioral support, observation, skill building, and implementation of individualized service or treatment-related supports in accordance with facility policy and resident needs. The DSP is a frontline direct care role. The employee is expected to interact with residents in a manner that supports dignity, privacy, choice, safety, stability, and community inclusion. The DSP carries out day-to-day resident support tasks under the direction of facility leadership and under paraprofessional supervision requirements established by the organization. Role Purpose The purpose of this position is to provide direct care and supervision to residents in the home; support implementation of resident routines, goals, and service-related interventions; maintain a safe and therapeutic home environment; document resident observations, services, and significant events accurately and timely; communicate resident needs and concerns to leadership and supervising staff; and promote resident rights, confidentiality, and recovery-oriented support in daily practice. Direct Resident Care and Supervision Supervise residents in the home and community as assigned. Monitor resident safety, emotional stability, behavior, participation in daily routines, and general functioning. Provide support during waking, evening, overnight, weekend, and holiday shifts as scheduled. Promptly report changes in resident condition, safety concerns, or behavioral concerns to the Clinical House Manager and/or leadership. Help maintain calm, structure, consistency, and a therapeutic tone within the home. Serve as the available on-duty staff member when scheduled and appropriately trained. Support With Daily Living and Independent Living Skills Assist residents with establishing and maintaining healthy daily routines. Encourage personal responsibility and age-appropriate independence. Support meal routines, housekeeping participation, laundry, and room upkeep as appropriate. Reinforce coping skills, structure, and healthy daily habits. Encourage participation in treatment, appointments, wellness activities, and recovery-supportive routines. Behavioral Support and Recovery-Oriented Interventions Provide supportive redirection, encouragement, and structure using respectful and trauma-informed approaches. Use approved de-escalation strategies and behavior-support techniques directed by leadership or resident plans. Reinforce resident goals, wellness strategies, and house expectations. Communicate observed triggers, strengths, and behavioral patterns to supervisors. Maintain professional boundaries while building supportive rapport with residents. Documentation and Shift Communication Complete shift notes, communication logs, house checklists, and required forms in a timely and legible manner. Document resident participation, significant observations, incidents, refusals, concerns, and follow-up actions. Communicate relevant information during shift change and to supervisors promptly. Report incomplete, inconsistent, or missing documentation to the appropriate supervisor. Protect confidential resident information in verbal, written, and electronic communication. Medication Support and Medication-Related Responsibilities Assist with medication-related tasks only within the limits of facility policy, training, competency, and legal authorization. Follow facility medication procedures exactly when assigned medication-related duties. Observe and report missed doses, refusals, side effects, or medication concerns immediately. Complete MAR-related documentation as required when properly trained and authorized. Communicate medication refill needs or medication-related concerns to the Clinical House Manager or other designated staff. Never administer medication outside the scope of training, authorization, and facility policy. Safety, Emergencies, and Incident Response Follow emergency procedures, evacuation instructions, and all resident safety protocols. Participate in fire drills, disaster drills, and other preparedness activities. Respond calmly and appropriately to accidents, medical concerns, behavioral crises, and urgent situations within training and role limits. Notify supervisors and emergency personnel as required. Document incidents, injuries, unusual occurrences, and follow-up actions promptly. Monitor the environment for hazards and report safety concerns immediately. Community Inclusion, Transportation Support, and Appointment Support Support resident attendance at appointments, community outings, and treatment-related activities. Assist with transportation readiness and transport as assigned and authorized. Support resident participation in recreation, wellness, and community-based opportunities. Encourage appropriate contact with family or legally responsible persons when authorized. Communicate appointment outcomes or follow-up needs to supervisors. Housekeeping, Meal Support, and Home Environment Maintenance Assist with meal preparation, meal monitoring, and kitchen cleanup as assigned. Help maintain shared spaces in a neat and sanitary condition. Encourage resident participation in housekeeping tasks appropriate to their needs. Monitor supplies and report low inventory or environmental concerns. Report maintenance problems, safety hazards, or sanitation concerns promptly. Rights, Confidentiality, and Professional Conduct Maintain confidentiality of resident information at all times. Treat residents respectfully and without discrimination. Follow all policies related to boundaries, abuse and neglect reporting, grievances, and professionalism. Participate in required orientation, in-service training, and continuing education. Report suspected abuse, neglect, exploitation, or policy violations immediately through required channels. Other Duties The Direct Service Provider performs other related duties as assigned to support safe residential operations, continuity of care, staff teamwork, compliance, and resident well-being. Minimum Qualifications At least 18 years of age. Able to read, write, understand, and follow directions. Meets the minimum education, competency, work experience, skills, and other qualifications established by the facility for the position. Has no substantiated findings of abuse or neglect on the North Carolina Health Care Personnel Registry. Holds any current license, registration, or certification required for duties actually performed. Generally possesses a high school diploma or GED, unless otherwise permitted by applicable rule or prior employment exception. Preferred Qualifications Experience in behavioral health, residential care, direct support, group homes, assisted living, human services, or community-based services. Current CPR, first aid, seizure-management, and airway-obstruction training. Valid driver's license and acceptable driving record if transportation is part of the role. Medication training if medication-related duties will be assigned. Required Supervision Because this role falls within the paraprofessional/direct care staff category, the DSP shall be supervised by an Associate Professional or Qualified Professional with the population served.
