We are seeking a dedicated AFL (Adult Family Living) Provider to join our team. As an AFL Provider, you will play a crucial role in providing care and support to individuals with developmental disabilities within a home community setting. Your compassion and commitment will directly impact the quality of life for those under your care.
Duties:
Deliver personalized care to residents in accordance with HIPAA regulations and Resident Rights.
Provide direct support to individuals with developmental disabilities, ensuring their well-being and safety.
Collaborate with a multidisciplinary team to create individualized care plans.
Assist residents with activities of daily living, including personal hygiene, meal preparation, and medication management.
Maintain a clean and organized living environment conducive to the residents' needs.
Monitor and report any changes in residents' health or behavior to the appropriate healthcare professionals.
Foster a supportive and inclusive atmosphere that promotes independence and dignity for all residents.
Qualifications:
Experience in assisted living, nursing homes, or long-term care facilities is preferred.
Knowledge of working with people with developmental disabilities.
Familiarity with systems related to care provision for individuals with special needs.
Strong communication skills and the ability to work effectively in a team environment.
Compassionate nature and a genuine desire to make a positive impact on others' lives.
