This role is essential in ensuring that the residents receive personalised, high-quality care. The ideal candidate will serve as a liaison between residents, their families, and our care giving team, ensuring that all care needs are met. Responsibilities
- Coordinate and monitor care plans for individuals requiring assistance with daily living activities
- Conduct assessments to determine the level of care needed by each client
- Collaborate with healthcare providers, social workers, and family members to develop comprehensive care plans
- Provide guidance and support to caregivers on proper techniques for assisting clients
- Monitor vital signs and behaviour management of clients
- Ensure compliance with all regulations and standards related to care services
- Maintain accurate and up-to-date documentation of client information Qualifications
- Must be a LPN Certified for North Caroline State.
- Experience in assisted living, senior care, care giving, or related fields
- Knowledge of behaviour management techniques and vital signs monitoring
- Familiarity with group home operations, social work principles, and long-term care facilities
- Strong communication and interpersonal skills to interact effectively with clients, families, and healthcare professionals
- Ability to work collaboratively in a team environment to provide optimal care for clients
- Certification or degree in a relevant field such as nursing, social work, or direct support is preferred
