AFL Provider

Overview:
We are seeking a dedicated AFL (Adult Family Living) Provider to join our team. As an AFL Provider, you will play a crucial role in providing care and support to individuals with developmental disabilities within a home community setting. Your compassion and commitment will directly impact the quality of life for those under your care.
Duties:
  • Deliver personalized care to residents in accordance with HIPAA regulations and Resident Rights.
  • Provide direct support to individuals with developmental disabilities, ensuring their well-being and safety.
  • Collaborate with a multidisciplinary team to create individualized care plans.
  • Assist residents with activities of daily living, including personal hygiene, meal preparation, and medication management.
  • Maintain a clean and organized living environment conducive to the residents' needs.
  • Monitor and report any changes in residents' health or behavior to the appropriate healthcare professionals.
  • Foster a supportive and inclusive atmosphere that promotes independence and dignity for all residents.
  • We recieve various referrals that we "match" with your experience.

Qualifications:
  • Experience in assisted living, nursing homes, or long-term care facilities is preferred.
  • Knowledge of working with people with developmental disabilities.
  • Familiarity with systems related to care provision for individuals with special needs.
  • Strong communication skills and the ability to work effectively in a team environment.
  • Compassionate nature and a genuine desire to make a positive impact on others' lives.
