Care Team Coordinator Senior Helpers Fargo - 3.7 Fargo, ND Job Details Full-time $20 an hour 11 hours ago Benefits Paid holidays Health insurance Dental insurance Paid time off Vision insurance Qualifications Phone communication Newsletters (communication methods) Client interaction via phone calls Full Job Description The Care Team Coordinator is dedicated to continuity of care, quality outcomes, and caregiver support by building trusted relationships, providing guidance and mentorship to care team members, and ensuring every client receives reliable, compassionate care. Primary Responsibilities (including, but not limited to): Responsible for covering open shifts and/or last-minute callouts/no-shows during the work day. Introduces caregivers to clients. Conduct routine site checks of caregivers to ensure care plans are being implemented correctly, policies and procedures are adhered to, and all job duties are performed in a manner that is satisfactory and meets the Senior Helpers brand expectations. Confirms that clients and their families are satisfied with the Senior Helpers services and communicates findings. Demonstrate an ability to identify and solve problems; use initiative and sound judgment to make quality decisions. Maintain an excellent rapport with clients and caregivers and effectively promote positive interpersonal relationships. Distribution and management of the caregiver newsletter With direction from the Care Team Manager (CTM), review the daily logs to ensure that the caregiver is following the care plan. Assists CTM with New Hire Orientation (NHO) and training programs for caregivers. Participates in on-call rotation, including every other weekend and rotating holidays Other duties as assigned Qualifications Must have excellent organizational skills, attention to detail, and the ability to prioritize in a changing environment Must have excellent phone skills and follow-up skills with prospects, clients, and client families Proactive problem prevention and issue resolution leadership ability Industry experience preferred Bachelor's degree and one year of related work experience strongly preferred Strong verbal communication skills, especially using the telephone Ability to work independently and as part of a team Benefits include: Full Time Schedule Health Insurance 2 weeks PTO 401k Dental, vision and other supplemental insurance available through Aflac 6 Paid Holidays
