Band 6 Job overview Lincoln CRHT is seeking a skilled and motivated Acute Care Worker to join our community-based Crisis and Home Treatment Team within Lincolnshire Partnership NHS Foundation Trust. We are based at Peter Hodgkinson Centre but we offer a flexible approach to working life. You will play a key role in the assessment, coordination and delivery of high-quality, evidence-based mental health care to adults experiencing acute mental health crises. Working autonomously and within a multidisciplinary team, you will manage a team-held caseload, complete comprehensive assessments and risk management plans, and ensure effective care planning and discharge arrangements. The role includes working across Crisis Resolution and Home Treatment services, whilst also liaising with our inpatient services, local social care providers, police colleagues, AMHP and community-based services. As a senior practitioner, you will provide professional advice and clinical leadership within the team, support service users and carers to engage in care, and promote recovery and independence. This role requires flexibility to work as part of a 7-day service covering nights, days, weekends and bank holidays. We also offer an excellent opportunity to further develop leadership and specialist crisis care skills within a supportive and dynamic team. Main duties of the job Undertake comprehensive mental health assessments and risk assessments for adults experiencing acute mental health crises. Plan, implement and regularly review person-centred care plans in collaboration with service users, carers and the multi-disciplinary team. Manage a defined team held caseload, ensuring safe, effective and timely interventions and discharge planning. Deliver crisis care across CRHT services, including the Section 136 Suite, Triage Car and Rapid Response services. Liaison with our inpatient services, local social care providers, police colleagues, AMHP and community-based services. Promote recovery, independence and safeguarding in line with Trust policies and national guidance. Maintain accurate, timely and high-quality clinical records and documentation. Provide clinical leadership, professional advice and role modelling within the team. Deliver management and clinical supervision to Band 5 and Band 3 staff, supporting development and safe practice. Contribute to clinical governance, audit, service improvement and quality initiatives. Work flexibly as part of a 7-day 24-hour crisis service, inclusive of nights, weekends and bank holidays. RMN or RNLD or RN:

1st Level Registration (NMC) or degree or equivalent Mentorship Module (degree level ) Evidence of specialised continued professional training (degree level) in clinical practice Current Registered Professional ENB998 / FliPS or equivalent Desirable criteria Management Training Experience Essential criteria Relevant experience of working with adults with mental health needs. Or clear evidence of substantial experience of working with individuals in crisis and skills that are transferable to both the community/in-patient Experience of working with people with severe and enduring mental illnesses Evidence of continuing professional development Skills & Competences Essential criteria Sound knowledge of national agenda for mental health Highly developed clinical reasoning skills Sound knowledge of clinical/risk assessment and understanding of Information Governance principles Highly developed communication skills Delegation whilst maintaining overall responsibility for patients care, where appropriate Demonstrate the ability to lead a clinical team Highly motivated & able to engage with service users & carers to improve outcomes Ability to work independently and collectively.
