Job Description

Family Support refers to community-based services broadly intended to promote family and child wellness, safety and stability, improve family functioning and community engagement so that the family can sustain itself without department assistance. Family Support relies, in part, on the philosophy that within even the most difficult situations, families have identifiable strengths upon which they can build new skills and improve their circumstances.
Purpose:
Strengthen the family and prevent of family dissolution, or facilitate the return of children who've been placed out of home. Use an ecological approach, approach the interdependence of the child, family and broader community environment, rather than focusing solely on the youth or family.

  • Reducing disruption in home placement after discharge
  • Encouraging a culturally relevant, strength-based approach to all service delivered
  • Choosing validated or criterion-referenced measures where possible to document process and outcome variables and improve results as well as to stakeholders
  • Improve family functioning and preserve the integrity of the family
  • Encouraging and supporting follow-up with families
  • Linking the family to community supports Required Knowledge, Skills and Abilities Bachelor's Degree in Social Work or related field required.
Two years experience working with children and families preferred. Basic knowledge of Microsoft Office computer software and Electronic Health Record (EHR) system. Ability to communicate effectively, both verbally and in writing. Ability to maintain complete and accurate records. Ability to work with a diverse group of individuals. Ability to manage crisis situations. Ability to work with minimal direct supervision. Must be detail oriented with the ability to discern discrepancies. Knowledge of the theories of early child growth and development. Valid driver's license three points or less and ability to meet transportation needs of agency. Must complete a defensive driving course as sanctioned by the Nebraska Safety Council or similar agency. Have and maintain agency required amount of automobile full coverage insurance. Acquire and maintain certification in First Aid and Cardiopulmonary Resuscitation (CPR). Major Responsibilities Establish and maintain a therapeutic relationship with families to help them cope more effectively with the stresses of daily life by the introduction of new support systems, and by connecting them to community resources Coordinate times, locations and dates of family support sessions. Provide transportation when necessary to help the family meet stated family support goals. Teach & model parenting skills using an evidenced-based model. Provide introduction of parent-directed activities including daily-living activities such as curfew, consequences, and chores. Provide written progress notes to report the progress of the family towards stated goals. Be on time for family support sessions. Call the In-Home Services Coordinator if unable to maintain the family support session. Attend family team meetings and actively participate with the family team. Assist with the compliance of all applications Federal, State, local and agency regulatory laws, standards, and policies.
