Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by observing vital signs, patient behavior, and overall condition, reporting any concerns promptly Provide medication reminders and assist with medication administration following care plans and HIPAA regulations Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Utilize specialized equipment such as Hoyer lifts or transfer devices to ensure safe mobility and transfers Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces Support clients with cognitive impairments like Alzheimer's or dementia through memory care techniques and patient engagement strategies Skills Proven experience in home & community care, assisted living, or long-term care environments Knowledge of patient observation, vital signs monitoring, and basic nursing procedures including CPR and first aid certification Familiarity with working with individuals with developmental disabilities, autism, or dementia-related conditions Ability to perform heavy lifting safely using equipment like Hoyer lifts or resident's (personal assistance devices) Strong communication skills for effective interaction with clients, families, and healthcare teams Understanding of HIPAA compliance to ensure confidentiality and privacy of patient information Experience in meal preparation, medication management, and developing or following care plans This role offers a rewarding career path for compassionate caregivers eager to provide exceptional home health services.
