We specialize in working with clients who are living with Dementia. Some clients live in a senior care community while others live in their home. You will play a vital role in delivering high-quality patient-centered care, fostering trust, and promoting well-being in a supportive environment. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Observe clients for any changes in health or behavior, documenting findings accurately Ensure compliance with HIPAA regulations to protect patient privacy Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Support mobility and transfers of the client as needed Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces Experience Prior experience in home health care, assisted living facilities, nursing homes, or group homes is required Knowledge of patient observation techniques, medication administration, and vital signs monitoring is essential Familiarity with care plans development and implementation for individuals with complex medical or behavioral needs Experience working with individuals with disabilities or chronic conditions such as dementia or hospice care is a strong plus Demonstrated ability to work independently while maintaining excellent communication skills with clients, families, and team members Proven track record of providing compassionate direct support while adhering to safety protocols and privacy standards Proven track record of strong attendance and dependability Embark on a fulfilling career where your dedication transforms lives.
