What We Offer Flexible Scheduling Supportive and Collaborative Team Environment Meaningful One-on-One Patient Care A Culture Focused on Compassion, Excellence, and Respect Position Summary As a Home Health Occupational Therapist, you will help patients improve their ability to perform daily activities, enhance safety within the home, and maximize independence. You will develop individualized treatment plans and work closely with patients, families, physicians, and the interdisciplinary care team to achieve the best possible outcomes. Responsibilities Perform comprehensive evaluations of patients' physical, cognitive, and functional abilities Develop individualized treatment plans based on patient needs and goals Implement therapeutic interventions to improve activities of daily living and overall independence Educate patients and family members on home safety, adaptive techniques, and use of assistive equipment Monitor patient progress and modify treatment plans as needed Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate care Maintain timely and accurate documentation in accordance with home health regulations and agency standards Complete discharge planning and provide recommendations for continued success at home Qualifications Required Bachelor's or Master's Degree in Occupational Therapy from an accredited program Current Nebraska Occupational Therapist license in good standing Current driver's license, reliable transportation, and auto insurance Strong communication and interpersonal skills Ability to work independently while maintaining effective collaboration with the care team Preferred Previous Home Health experience but not required.
