Temporary, Part-time, Full-time Position Summary:
Personal Care Services (PCS) Caregiver The PCS Caregiver provides essential non-medical, in-home support to individuals enrolled in the New Mexico Medicaid Centennial Care program. You will deliver services based on the client's authorized Plan of Care, focusing on Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Our goal is to empower Albuquerque residents to live independently and with dignity in their own homes while ensuring full compliance with New Mexico Administrative Code (NMAC) and agency standards. Essential Duties and Responsibilities Caregivers provide one-on-one support as outlined in the client's specific Plan of Care, including:
Personal Care Services:
Assist with bathing, dressing, grooming, oral hygiene, and skin care. Provide support with toileting, bowel/bladder care, and safe transfers or ambulation using proper body mechanics.
Homemaker & Support Services:
Perform light housekeeping, laundry, and linens; assist with grocery shopping, errands, and meal preparation (including specialized diets as required).
Health-Related Support:
Provide medication reminders, assist with self-administered medications, and monitor for changes in physical or mental status.
Documentation & Safety:
Maintain accurate daily logs of services provided; report any signs of neglect, abuse, or significant health changes to the supervisor immediately.
Infection Control:
Adhere to universal precautions and sanitary standards to ensure a healthy environment for both the client and the caregiver.
Qualifications Eligibility:
Must meet New Mexico Medicaid (HSD) caregiver eligibility requirements.
Background:
Ability to pass a mandatory New Mexico Caregivers Criminal History Screening (CCHSP) and Employee Abuse Registry check.
Education:
High school diploma or GED preferred; must be able to read, write, and follow a written Plan of Care in English (Bilingual Spanish/English is a plus in the Albuquerque area).
Reliability:
Must have reliable transportation to travel to various residential locations across the Albuquerque metro area.
Age:
Must be 18 years of age or older.
Soft Skills:
Must demonstrate a patient, empathetic, and professional demeanor when working with elderly or disabled populations. Physical Requirements Ability to stand, walk, bend, reach, and kneel for extended periods. Ability to lift, push, or pull up to 50 lbs (or agency-specified limit). Ability to assist clients with various levels of mobility using assistive devices (gait belts, walkers, etc.).
Work Environment Setting:
Private residences and community settings throughout Albuquerque and surrounding Bernalillo County.
Conditions:
Exposure to various home environments, including pets and varying temperatures.
