Job Description

Job Summary:
The Care Facilitator works efficiently and directly with Consumer/Member in the admission process for the Private Services division of the Agency. They are responsible to ensure the provision of care of the consumer/member in all aspects of the program.
Requirements:
Must have a clean criminal background check. Assure HIPAA compliance at all times. At least a high school diploma. A minimum of 1 year of experience working in an office environment. Ability to provide proof of right to work in the United States. Must have a current driver's license, proof of automobile insurance, satisfactory driving record and reliable transportation. Willing to travel as deemed necessary.
Duties and Responsibilities:
a) Consumer/Member Responsible for reporting Consumer/Member concerns to appropriate staff and follow-up. May serve as an interpretive liaison between Third Party Assessors and Consumer/Member per Consumers/Members request. Perform supervisory visits to Consumer/Member per State Program guidelines. Assist with internal and external transfer process. Ensures the continuity of care between the Managed Care Organization (MCO) and Consumer/Member. Complete eligibility checks on the first and fifteenth business day of every month and notify Client, Branch Coordinator, and Biller of ineligibility. Assist consumer/member with the Medicaid application process. b) Subordinate Staff Assist in orientation of new employee to Agency in the absence of the Branch Coordinator Assist in ensuring Caregivers are compliant with the IPoC/Personal Care Service Plan. Conducts CPR/AED & First Aide training to caregivers. c) Documentation Ensure accuracy of documentation prior to entering and filing. Responsible for accurately documenting changes in Consumer/Member information. Responsible for the completion of Incident Reports, (i.e., entry, updating, follow-up, etc.) d) Department Verify Consumer/Member eligibility and authorizations prior to admission and thereafter as directed by State Program Guidelines. Attend scheduled department meetings in person or via Zoom. e) Agency Participate in On-Call rotation. Assist in preparation for State and Agency audits. Attend in-state continuing education classes/conferences
