Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe changes in health status to report promptly to healthcare professionals Administer medications following care plans and ensure compliance with HIPAA regulations for patient confidentiality Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary Use specialized equipment such as Hoyer lifts or other assistive devices to safely transfer clients Maintain accurate documentation of care provided, patient observations, and any incidents or concerns Support clients with memory care needs, including dementia or Alzheimer's care, through engaging activities and a safe environment Experience Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred Demonstrated knowledge of patient care procedures, medication administration, and vital signs monitoring Experience working with individuals with developmental disabilities, autism spectrum disorder, or dementia is advantageous Familiarity with HIPAA regulations and maintaining patient confidentiality in a home setting Hands-on experience with emergency response procedures such as CPR and first aid certification is desirable Ability to perform heavy lifting safely and operate mobility aids like Hoyer lifts or wheelchairs when necessary Strong communication skills for collaborating with families, healthcare providers, and social workers Join us in delivering compassionate, professional care that transforms lives.
