A CSW ensures the well-being of clients in all of their environments and promotes their development of independent skills. A key component of this job is to assist clients in their environmental, social, emotional and daily life skills development. This increases their independence and allows them to function appropriately in the community.Role Responsibilities :
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Provides support to adults, adolescents and children. Services provided include crisis intervention, behavior management, and life skills training, whether on a one-to-one basis or in a group environment Plans, develops, implements and evaluates recreational, social and/or educational activities for clients served by the organization Create a full and complete treatment plan (please refer to Employee Handbook) with the patient and/or caregivers. The treatment plan will need to be re-evaluated and sign by client and/or caregiver's every 90 days Create and maintain accurate and current documentation of each session through the TTC electronic medical records system. The session note will be completed within 48 hours of when the session was conducted. The CSW will sign the note for the supervisor to review and approve. The supervisor will then sign the note if the note is accurate and complete Aids clients in their own homes and in the community to allow them to function more safely and independently. Facilitates recreational, social and educational activities within the clients home environment when possible. Identifies social, economic, recreational and educational services in the community that will meet clients' needs, Maintains liaison with other agencies, professionals, government officials and the community Recognizes potential crisis situations, analyzes such situations accurately, develops strategies to deal with such situations, and informs their Supervisor when such incidents arise Recognizes potential emergency situations, analyzes situations accurately, develops strategies to deal with such situations i.e. dispensing medication, dealing with client's aggressive behavior to ensure no harm comes to the client, staff and/or the public Assists clients with daily life skills and/or behavior management training It is the responsibility of the Worker to ensure that all requirements are met to provide safe transportation to community visits i.e. proper driver's license, brake check Monitors clients' progress and well-being and assists case management team in evaluating the effectiveness of planned counseling and support. Provides suggestions for the modification of clients' program plans Maintains written reports regarding clients' daily activities and progress. Ensures that all required documentation is complete and accurate Reviews and evaluates clients' progress and makes adjustments to programs as required Encourages and facilitates the participation of individuals in the program. Creates a fun and enjoyable atmosphere in order to enhance the program's appeal to clients. Provides liaison with other agencies and the community. Fosters positive working relationships with staff members. Provides input to management with regard to the development of the program and participates in program evaluations. Will check-in at TTC facility at the start of shift and travel in Rio Rancho & Albuquerque. Must have and maintain valid New Mexico driver's license and clean driving record. Must have and maintain reliable vehicle and vehicle insurance. Travel Time must be pre-approved by Office manager or CCSS Supervisor. Performs other related duties as required.Organizational Structure :
TTC is governed by its CEO and COO who oversee the corporation as a whole. TTC houses multiple departments. Each department has its own Director and Assistant Director (when necessary). Individual Clinicians may refer to their contracts for who their direct supervisor is and when applicable who their supervisees are. CSW's are direct care providers within the organization. They are supported by an administrative and supervisory staff, but they remain the primary face of the organization. It is important that CSW's maintain consistency, professionalism and kindness when interacting with other staff and clients.Supervision Requirements:
Each Clinician is required to attend individual supervision as stipulated within their individual contract. CSW's are required to keep their supervisors appraised of all client's progress, struggles or significant incidents, ethical or legal dilemmas involving clients, colleagues or administration. The status of their own mental and emotional health over time in working with this population so the supervisor may work with the therapist to inform the therapist of resources, develop a plan for recovery and prevent burn out. Reflective consultation is required as stipulated within individual contracts. Quarterly supervision goals should be evaluated and supported by the supervisor. Annually, Clinicians will receive an evaluation to support their growth and goal attainment. Evaluations are also provided to redirect Clinicians who are not meeting expressed expectations within this job description and/or TTC policy and procedures.Training Expectations :
Each CSW is expected to fulfill all continuing education requirements for all current licenses and certifications. It is expected that ongoing training will support their trauma focus at TTC as well as increase their scope of practice.Job Type:
