Job Description
Health Home Case Manager About Us KidsPeace is a private charity dedicated to supporting the behavioral and mental health needs of children, families, and communities. Since 1882, we have offered psychiatric hospital services, residential treatment programs, accredited educational services, and foster care programs. Our team works in an environment defined by compassion, creativity, collaboration, and a commitment to delivering hope and healing. Position Overview Care Managers provide outreach and enrollment services to children who are presumed to meet eligibility requirements for New York State's Children's Health Home program. They conduct assessments and complete documentation to confirm eligibility and identify areas where supportive services are needed. Care Managers work collaboratively with children, families, and service providers to develop a comprehensive Plan of Care, identify additional services as appropriate, and coordinate ongoing involvement of all parties to ensure effective implementation of the plan. They also monitor and document progress, facilitate transitions between health homes when necessary, and support movement to other levels of care when eligibility changes. - A team environment built on collaboration, respect, and compassion Key Responsibilities Complete accurate and timely documentation in accordance with program and regulatory requirements Collaborate with children, families, and service providers to develop and maintain a comprehensive Plan of Care Identify, refer, and coordinate additional services and supports as needed Facilitate ongoing communication and collaboration among families and service providers to ensure effective service delivery Monitor, document, and evaluate progress toward care plan goals Support transitions between health homes or to other levels of care when eligibility or service needs change Maintain compliance with agency policies, state regulations, and confidentiality standards Qualifications Bachelor's degree with two (2) years of relevant experience , or Master's degree with one (1) year of relevant experience Valid Driver's License
