Job Description

Metro Children Services is seeking compassionate, reliable, and motivated professionals to provide Caregiver/Family Advocacy & Support Services (CFASS) to children, youth, and families enrolled in New York State Home and Community Based Services (HCBS). CFASS enhances the ability of children and youth with developmental, physical, behavioral, and emotional disabilities to function successfully within their family and community while strengthening the caregiver's ability to provide support, advocacy, and effective care. Providers serve as a bridge between families, community resources, and service providers to ensure families feel supported, informed, and empowered. As a CFASS Provider, you will work directly with children, youth, caregivers, and families to provide advocacy, education, skill-building, and support services. You will help families navigate community resources, strengthen caregiving skills, promote self-advocacy, and support the child or youth in achieving goals identified in their HCBS Plan of Care. Responsibilities Provide individualized support based on the participant's HCBS service plan. Help caregivers and families understand available resources, services, and community supports. Advocate on behalf of children, youth, and families when accessing services and navigating systems. Support families in building self-sufficiency and confidence in caring for their child. Assist caregivers in developing advocacy, problem-solving, and goal-setting skills. Provide educational, advocacy, and support resources to families. Train children, youth, and caregivers on methods and strategies that promote success in the home and community. Support self-advocacy and community integration skills. Educate caregivers regarding their child's developmental, behavioral, emotional, physical, or medical needs. Provide guidance during transitions and other significant life changes. Maintain effective communication with families, caregivers, schools, Care Managers, and other service providers. Collaborate with the HCBS team to ensure quality service delivery. Complete professional and timely session notes for each service provided. Document progress toward goals, participant engagement, and the need for continued services. Maintain compliance with all Metro Children Services, Medicaid, and New York State Children's HCBS requirements. Maintain confidentiality and professional boundaries at all times. Attend required trainings and supervision sessions. Accept and apply constructive feedback to improve service quality and professional growth. Pay:
$20.00
  • $35.00 per hour Application Question(s): Which service(s) are you interested in providing? You may list more than one. CFASS Level 1
  • Requires a High School Diploma/GED and experience working with children, youth, families, or in a related human services field. CFASS Level 2
  • Requires a Bachelor's Degree and at least two (2) years of experience working with children, youth, families, or in a related human services field. Respite DSP
  • Requires experience providing direct support, supervision, or care to children and/or individuals with developmental, behavioral, emotional, or physical disabilities.
