Provide care and support to residents by following individualized assistance plans.

Compensation & Schedule Pay Rate:

$19.15 an hour

Shifts:

Full-time and part-time opportunities available Responsibilities Assist residents with personal care activities Support with light housekeeping and daily tasks Ensure resident safety, including proper use of equipment (wheelchairs, oxygen, side rails, etc.) Accurately document services in accordance with agency policies Communicate any changes in a resident's condition to the RN Supervisor promptly Collaborate with the healthcare team as required by NYS regulations Maintain confidentiality of resident and family information Build positive relationships with residents and their families Follow dress code and maintain professional hygiene standards Demonstrate reliability, punctuality, and professionalism Report concerns regarding resident well-being to the Case Manager or Administration Perform additional duties as assigned by the supervising nurse
