Job Description

Job Summary We are seeking a dedicated and compassionate Direct Care Professional to provide exceptional support and care to individuals with diverse needs. This role offers an opportunity to make a meaningful difference in the lives of residents, helping them maintain independence, dignity, and quality of life. The ideal candidate will possess a strong background in caregiving, with experience in assisted living, long-term care, or home and community care environments. Your primary responsibility will be to deliver personalized assistance, ensuring safety, comfort, and well-being for those in your care. Duties Monitor the health and well-being of clients, ensuring their safety and comfort at all times. Implement and follow individualized care plans tailored to each resident's needs. Administer medications as prescribed, ensuring compliance with medical guidelines. Observe and document patient behaviors and progress, reporting any significant changes to the appropriate personnel. Uphold resident rights by treating all individuals with respect and dignity. Assist clients with daily living activities, including personal hygiene, meal preparation, Maintain a clean and safe environment, and transportation. Facilitate community integration by supporting clients in participating in social and recreational activities. Maintain confidentiality of client information in accordance with HIPAA regulations. Collaborate with other team members to create a positive living environment within group home settings or community care facilities. Experience Prior experience working in assisted living facilities, nursing homes, or personal care homes and caring for individuals with developmental disabilities. is preferred but not required. Proven ability to manage behavioral challenges through effective behavior management strategies. Knowledge of patient monitoring techniques and care plan implementation is beneficial. Familiarity with medication administration protocols is a plus. Strong observational skills to recognize changes in client behavior or health status. Understanding of resident rights and commitment to advocating for those rights. Ability to work effectively as part of a team while also being self-motivated. Excellent communication skills, both verbal and written, to interact effectively with clients, families, and team members.
