Northeast Ohio Hospice is seeking a dedicated and compassionate full-time STNA to join our hospice team. The position will cover a caseload split between Olmsted Falls and Chagrin Falls. As an integral member of our interdisciplinary team, you will provide personal care to patients facing end-of-life challenges and support their families during this sensitive time. Responsibilities Provide personal care services with compassion, dignity, and respect Assist patients with bathing, grooming, dressing, toileting, feeding, and other activities of daily living Observe and report changes in patient condition, symptoms, and emotional status to the clinical team Support patient comfort and safety according to the plan of care Communicate effectively with patients, families, facility staff, and the hospice interdisciplinary team Complete timely and accurate documentation within the EMR Participate in team collaboration and patient care coordination Qualifications Current STNA certification in the State of Ohio Minimum of 1 year of STNA experience preferred Hospice, long-term care, or home health experience preferred Compassionate and empathetic approach to patient care Strong communication and interpersonal skills Ability to work independently while collaborating within a team environment Valid driver's license, reliable transportation, and active auto insurance Experience with EMR documentation systems preferred Schedule & Benefits Full-time position Monday - Friday Primarily facility-based patient assignments with manageable travel expectations Mileage reimbursement at $0.70 per mile Supportive leadership and team environment Opportunities for professional growth and education Meaningful work that makes a difference in the lives of patients and families We are looking for team members who lead with compassion, treat patients like family, and are passionate about providing comfort and dignity at end of life.
