In this vital role, you will provide personalized in-home care, ensuring comfort, safety, and independence for individuals across a range of needs. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Perform patient observation to detect changes in condition and communicate findings promptly to healthcare teams. Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring the overall safety of the home setting. Support patients' emotional well-being through compassionate communication and engaging activities tailored to their needs. Work collaboratively with other caregivers and healthcare professionals to coordinate comprehensive home & community care services.
