Job Description
Provides direct care services to individuals with developmental disabilities. Driving a shuttle/van is required daily for this Job Description. Provides assistance, service and support to individuals. Utilizes a flexible work schedule to meet individual wants and/or needs in a variety of environments to expand opportunity for individuals' personnel growth. Participates in assuring community access for the individual through facilitating transportation options so as to avoid exclusion on the basis of lack of transportation. Maintains legible and accurate documentation in order to monitor effectiveness and efficiency of service delivery. Completes report forms as appropriate to satisfy record keeping regulations for various entities. Participates in the Individual Plan process as directed by Supervisor. Maintains awareness of the safety of the individual, co-workers and self. Must be able to lift 50 lbs. Creates and executes implementation plans for individuals as approved by Supervisor. Selects appropriate training techniques based upon individual learning styles and as directed by the training environment so as to assure optimum individual gain. Executes continuous training and guidance based upon the wants and/or needs of the consumer to assure long term support of the individual. Completes and maintains appropriate records so as to monitor individual progress/activities. Plays a visible role in the organization's public relations effort through substantial contact with individuals, their families and the general public. Develops positive relationships with individuals and, when applicable, their families. Improves the image of the organization while promoting the profession of providing supports for people with developmental disabilities. Continually accumulates information which enables better job understanding and performance. Attends staff meetings to stay abreast of current activities in the organization and the field of developmental disabilities. Participates in in-service training sessions which provide up-to-date and/or advanced performance techniques and information for personnel. Interacts with co-workers in a steady exchange of useful information and effective techniques. Performs miscellaneous duties to assist in maintaining services under all conditions. Maintains awareness of opportunities for the community participation of individuals. Reports unusual, dangerous, or otherwise significant events concerning the consumer and takes appropriate actions. •Year end bonus•
