Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals Administer medications accurately following care plans and HIPAA privacy regulations Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary Support clients with memory care, dementia, Alzheimer's, or other cognitive impairments through engaging activities and supervision Use specialized equipment such as Hoyer lifts or resident lifts to ensure safe transfers and mobility assistance Maintain a clean, safe, and comfortable environment by performing light housekeeping tasks including cleaning and laundry Document care activities meticulously in compliance with healthcare standards and care plans Skills Proven experience in home & community care, assisted living, nursing homes, or long-term care environments Knowledge of patient observation techniques and ability to monitor vital signs effectively Familiarity with HIPAA regulations to ensure client confidentiality at all times Strong skills in medication administration, meal preparation, and assisting with ADLs Experience working with individuals with developmental disabilities, autism, or dementia-related conditions such as Alzheimer's or memory care needs Ability to perform heavy lifting safely using equipment like Hoyer lifts when required Excellent communication skills for collaborating with clients' families, healthcare providers, and team members Certifications such as CPR, First Aid, and relevant nursing or home health training are highly desirable Join us in delivering exceptional in-home care that promotes dignity, independence, and well-being.
