Job Description
JOB SUMMARY
The Home Care RN Supervisor is responsible for ensuring that patient care is coordinated and managed e appropriately according to state and federal guidelines. Position will ensure that patient and family care services are delivered appropriately and for the supervision of home care medical and support staff. The Supervisor will be a resource person by providing subject matter expertise and quality practices for home care services and provide education and training as applicable. Team member will be expected to review and monitor operations and clinician activity reports and address areas for improvement for a plan of action.RESPONSIBILITIES
(ESSENTIALFUNCTIONS
): Responsible for the day-to-day operations and assist with scheduling of all field clinicians under their supervision. Provides guidance and assistance to team members in all matters relating to the delivery of Home Care and Hospice services to assure that regulatory, quality, and practice standards are maintained. Assists clinicians with maintaining a safe, patient-family centered care. Develops and cultivates collegial working relationships with physicians/providers, leadership team members, and community partners. Prepares and leads case conference/IDT meeting with Home Care and Hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. Reviews and manages case referrals by ensuring they are appropriate to the service line and meet eligibility requirements. Assists in the formulating, revising, implementing and evaluating organization policies, procedures, in-services, goals and objectives. Assists with identification, measuring, and analyzing audit process to ensure continuous improvement and safeness in clinical care. Communicates outcome data and provides education/training to organization personnel, as applicable. Assists in the screening and interviewing process of new team members. Onboards and mentors new team members, as applicable. Promotes the mission, vision, and values of the organization. Available at all times during operating hours or ensures alternate backup is available. Participates in cross-covering duties for alternate Home Care/Hospice Nursing Supervisor, as needed. Participates in clinical and/or administrative on-call, as required. Participates in leadership and committee meetings, as assigned. Reports to and assist the Manager in overall management of the department. Regular attendance and punctuality for scheduled shifts. Appropriately adapts assessment, treatment, and/or care methods to accommodate the physical, cultural, age-specific, and other developmental needs of each person served. Maintains professional and technical knowledge through continuing education opportunities including internal and external educational offerings. Utilization of assistive devices for lifting is mandatory. Per DRH policy, all required conditions of employment must be met and maintained including required vaccinations. This position requires respirator and fit testing. Required testing will be performed on all team members at risk for exposure to airborne infectious diseases, droplet producing procedures, or any caustic chemicals. Must adhere to, and promote, safety protocols at all times. Implement DRH Standards of Behavior and exhibit behaviors consistent with DRH core values. Performs other related duties as assigned.JOB REQUIREMENTS
Minimum Qualifications:
Prior nursing experience in Home Care or Hospice setting required. At least 2 years of prior leadership experience in Home Care or Hospice setting preferred. Familiarity or experience with QAPI process preferred. Communication skills including fluency in oral and written English. Basic computer skills including the ability to send/receive/email, navigate information technology associated with the position, and use Electronic Health Record information tools. Excellent written, verbal, and interpersonal communication skills; Ability to remain flexible to quickly adapt to urgent situations. Ability to adapt procedures, processes, tools, equipment, and techniques to accomplish the requirements of the position. Advanced, fingerprint background checks are required for this position. Education and/orExperience:
Associate or Baccalaureate Degree in Nursing from a program accredited by the Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education (ACEN) or Commission for Nursing Education Accreditation (CNEA). Familiarity with local, state, and federal Home Care and/or Hospice general process, Conditions of Participation, and claims processing requirements. Strong professional work ethic and team player with effective critical thinking skills with ability to work within an interdisciplinary setting. Demonstrated excellence in observation, good nursing judgment, and communication skills. Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to medical providers, leadership team, and public groups. Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course to determine appropriateness of admission and assist with an effective plan of care.Certifications, Licenses, Registrations:
Current, unencumbered RN license in the State of Oklahoma or multistate licensure in a party state in compliance with primary residency rules and requirements. American Heart Association BLS required; May have up to 30 days from date of hire or transfer to obtain. NIHSS certification preferred. A current valid driver's license and automobile liability insurance must be maintained. Safety-Sensitive Position - This is a "Safety-Sensitive" Position as defined by Oklahoma law.
