SUMMARY:
Habilitation Training Services include evaluation, assistance, and training in self‑care, socialization, daily living, and companionship services. An HTS is a branch of a broader career known as Home Health Aid. The Habilitation Training Specialist works specifically with individuals with a cognitive disorder or low IQ. This position assists in caring for individuals with cognitive disorders by providing personal assistance, emotional support, and personal care. An HTS must document work on training goals established in a care plan for the individual. Such a care plan may include items such as cooking, self-care, and working with money skills. Training activities vary according to the abilities of the individual receiving services. An HTS may work part-time with one individual or full-time with one or more individuals.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Responsible for meeting all needs of assigned individual(s), which includes but is not restricted to feeding, toileting, bathing, lifting, socialization, preparing meals, and helping residents to choose proper dress. Responsible for understanding, implementing, and properly documenting all objectives and strategies derived from the Individual Plan (IP) to achieve stated goals. Documentation should be done at the time of the task. Responsible for following a weekly planned schedule of activities for the individual. Documentation of these activities and results. Responsible for properly reporting any accidents or incidents within 24 hours of occurrence. A handwritten statement is to be submitted to the Residential Coordinator on the same day, or if not serious, the next working day. Responsible for completing data collection, progress reports, and any other necessary documentation as it occurs. Responsible for always providing emotional support through physical presence and therapeutic interactions with residents throughout the shift. Responsible for assisting residents who are out of control or who may be experiencing a crisis, following their IP or PIP interventions. Responsible for ensuring residents' environmental safety according to established procedures. Responsible for notifying the Residential House Manager verbally and documenting any change in residents' condition or behavior, and/or any change in medical orders or medical concerns. Attendance of all staff meetings. Responsible for maintaining a clean and safe home. Responsible for administering medication in proper dosages and at the designated times. Responsible for daily medication count, which should be completed at the beginning of the shift and must properly fill out the med log at the time medication is administered. Responsible for adhering to and complying with all applicable Mujang's Care policies and procedures. Responsible for reporting any equipment failure or malfunctions to the Residential Coordinator. Responsible for other assigned tasks as designated by Residential House Manager or Program Coordinator.
QUALIFICATION REQUIREMENTS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. This position may require the use of a personal vehicle.
