Job Description

Conifer House a residential care facility is hiring a Resident Care Manager. The RCM will use their knowledge and skills every day to provide excellent care, as well as lead our talented and diverse care team. We look forward to introducing you to this wonderful group!
OVERVIEW OF POSITION
As RCM, you will work closely with families, staff, and outside providers. The RCM is considered a leader in this community-based care setting. The care team will be taught how to provide high quality care to our residents thanks to the guidance and coaching you provide. You will promote positive outcomes for each of our residents using the diverse knowledge and relational skills you bring with you.
QUALIFICATIONS
  • Two years previous experience and basic competencies working in a community-based care setting as a caregiver and med-tech are essential.
  • Comfortability and experience in determining resident health needs (this is an assessment skill we highly covet and actively look for in a good candidate)
JOB DUTIES
  • Maintaining an efficient care team by teaching basic health care skills including medication administration, alert charting, reviewing doctor's orders, reviewing lab test results, managing changes of resident conditions, reviewing incident reports, and all other aspects of resident care.
  • Coordination of quality care, working with the care team, doctors and families to maintain resident's care needs and all-around wellness.
  • Building relationships by bolstering our community culture alongside nursing staff, office personnel, residents and residents' family members, and all other individuals involved in resident care.
