America's Home Health Services (AHHS) is seeking an energetic, relationship-driven Home Care Liaison/Marketer to grow our presence throughout the Erie territory. This role is ideal for a motivated professional who thrives on building connections, developing referral partnerships, and promoting high-quality home care services within the community. Key Responsibilities:

Develop and maintain strong relationships with referral sources including Managed Care Organizations, hospitals, physician offices, rehabilitation centers, and community organizations Work with PA's Home and Community Based Waiver program Conduct regular in-person visits, presentations, and follow-ups with referral partners Identify new business opportunities and expand AHHS's footprint in the Erie market Educate potential clients and partners about AHHS services and value Track referral activity, maintain accurate records, and meet growth targets Collaborate with internal teams to ensure smooth onboarding of new clients Represent AHHS at networking events, health fairs, and community outreach initiatives Stay informed on industry trends and competitor activity

Qualifications:

Previous experience in home care marketing is a

MUST HAVE

Strong communication, networking, and relationship-building skills Self-motivated with the ability to work independently and meet goals Reliable transportation and willingness to travel throughout the Erie and the surrounding counties Knowledge of the local healthcare landscape is a plus Proficiency with CRM systems and
