Amada Senior Care Philadelphia, West Suburbs is seeking a Client Care Coordinator who thrives on connecting with people, supporting families, and ensuring exceptional care experiences for our clients and caregivers.
This role combines client relationship management, care coordination, case management, and community outreach. You'll serve as a trusted resource for clients, families, caregivers, and referral partners while helping ensure high-quality care and exceptional customer service. What You'll Do Manage a caseload of home care clients and support them throughout their care journey Complete client intakes, assessments, on-boarding, and individualized care planning Conduct supervisory visits, reassessments, and quality assurance check-ins to ensure client satisfaction and care plan compliance Build and maintain strong relationships with clients, family members, caregivers, referral sources, and community partners Coordinate caregiver schedules and client services to ensure continuity of care Monitor service authorizations, care plans, and compliance documentation to ensure records remain current and accurate Communicate regularly with families, caregivers, service coordinators, managed care organizations (MCOs), and healthcare professionals Follow up on missed visits, caregiver call-offs, client concerns, and service-related issues Educate caregivers on EVV, HHAeXchange, and AxisCare procedures and documentation requirements Support caregiver and client retention through proactive communication, problem-solving, and relationship-building Visit hospitals, rehabilitation centers, assisted living communities, physician offices, and other community partners to educate them about our services and develop referral relationships Participate in networking events, community outreach activities, and professional relationship-building opportunities Assist with incident reporting, audits, compliance requests, and operational support as needed What We're Looking For 2+ years of experience in home care, healthcare coordination, case management, client services, community outreach, sales, or a related field Previous experience managing client caseloads and coordinating services preferred Experience with HHAeXchange, AxisCare, EVV systems, or similar healthcare software is highly preferred Excellent communication, relationship-building, and customer service skills Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment Strong problem-solving abilities and attention to detail Compassionate, client-focused, and committed to delivering exceptional service Proficiency in Microsoft Office, Google Workspace, and basic computer systems Ability to work independently and as part of a collaborative team Valid driver's license, reliable transportation, and auto insurance required Comfortable traveling throughout the service area for client and referral visits Why Join Amada?
