Fox Chase Home Care is seeking a motivated and outgoing Client Intake & Outreach Coordinator to help grow our client base and connect families with the care services they need. This role combines intake coordination with outreach and relationship-building, making it ideal for someone who is confident making calls, educating potential clients, and creating positive first impressions. The ideal candidate is energetic, organized, personable, and comfortable speaking with people throughout the day while helping guide them through the process of starting home care services. Benefits 401(k) Health insurance Vision insurance Paid time off Referral program Growth opportunities within a supportive team environment What You'll Do Client Outreach & Relationship Building Conduct outbound calls to potential clients, referral sources, and community leads Educate individuals and families on home care services and available programs Build rapport and maintain professional relationships through strong communication Follow up consistently with interested prospects and referrals Help convert inquiries into active clients by guiding them through the intake process Intake Coordination & Service Setup Serve as the first point of contact for incoming service inquiries Collect and review client information to begin intake Verify insurance eligibility and required authorizations Ensure all intake documentation is complete and compliant Coordinate internally to transition approved clients into service setup efficiently What We're Looking For Energetic, outgoing, and personable personality Comfortable making high-volume outbound calls Strong communication and relationship-building skills Sales, customer service, outreach, intake, or healthcare experience preferred Ability to educate and engage potential clients confidently Strong organization and follow-up skills Ability to multitask in a fast-paced environment Bilingual English/Spanish is a plus Why Join Fox Chase Home Care?
