Job Summary :

We are seeking a caring and dedicated Habilitation (HAB) Specialist to join our team. The HAB Specialist will work closely with individuals with IDD to help them develop essential life skills, increase independence, and achieve personal goals. This role involves providing one-on-one support in areas such as communication, socialization, personal care, and daily living activities, while promoting dignity and respect.

Key Responsibilities :

Support individuals in learning social and communication skills to improve relationships and community interactions. Help individuals set and achieve personal goals related to self-care, work, and community participation. Implement and follow individualized habilitation plans, documenting progress and making adjustments as needed. Teach coping strategies and problem-solving skills to promote self-sufficiency. Encourage individuals to participate in community activities and social events. Ensure a safe, supportive environment that fosters learning and growth. Collaborate with family members, Program Specialists, and other staff to ensure coordinated support. Maintain accurate records, including daily progress notes, incident reports, and other required documentation. Advocate for individuals' rights and promote their full inclusion in the community.

Qualifications :

High School diploma or equivalent. Vehicle, Valid driver's license, registration, and insurance Ability to pass a background check Strong communication and interpersonal skills. Ability to work both independently and as part of a team. Patience, compassion, and a genuine desire to support individuals in leading fulfilling lives.
