Client (1) is a early 70s widowed female living with ALS (Lou-Gerious disease) in the Bethany area of Beaverton. She is living in her home with the help of family and an amazing in-home care team. I'm seeking a highly organized, detail‑driven Care Coordinator to keep the household running smoothly behind the scenes. This is a caregiving role but it is also an operations manager/care coordinator for a home. You'll be the person who keeps schedules aligned, uses clear professional communication to collaborate with other caregivers, monitors changes in care, orders supplies, and keeps track of small-but-important tasks. Also assists in keeping family updated and aware with clients care.
Schedule:
In-person caregiving Sunday & Monday 10am -3pm. Communication with team/family, hiring and other aspects of care coordination via phone during the week as needed. About 10-20 hours total (including in-person hours) depending on needs of client and team. What You'll Do Supply Management; Monitor levels of supplies, place orders, and anticipate need Appointment scheduling; assist client in scheduling doc appointments Care Advocacy; assist client in advocating the healthcare, attend appointments, set follow-ups Staff Coverage and Scheduling ; manage staff schedules (we have 3 caregivers) assist in care coverage to ensure support for Client Communication and Updates; relationship with family to be informed of any changes or updates in Clients care. Progress Monitoring; monitoring Lisa's care closely, documenting progression and care needs. Leadership and Team Support; Communicate with care staff to confirm shifts, updates, and changes, hiring and training new team members We have an organized system, and have had someone in this role for 7 yrs. We're not asking you to reinvent our process, but help keep it going, and implement changes where necessary.
