Home Care Provider Laconia, NH 03247 From $60,000 a year - Full-time, Contract From $60,000 a year - Full-time, Contract Community Integrated Services is seeking a compassionate Home Care Provider to welcome a client into their home and manage all aspects of care and supervision. This position requires that YOU have a spare bedroom available in your home for the individual, and the home must meet New Hampshire Certification standards. •Many of our clients have limited mobility. A wheelchair accessible home is a plus! As a Home Care Provider, you will have the opportunity to assist the person living with you to live a fulfilling, enjoyable life. Many of the supports that would be expected are helping with their daily living activities, such as building and keeping a day/evening routine, cooking, nutrition education, housekeeping, communication, appointment management, transportation and so many more life enriching activities! You will also work with a CIS staffing team to build and maintain a high quality of life for the person you bring into your home.
Responsibilities:
Manage varying levels of medical and behavioral needs. Administer care to ill, injured, or disabled clients. Monitor and report changes in patient health or behavior. Communicate with collaborating physicians or specialists regarding patient care. Coordinate individual medication administration. Assist Program Manager with development and implementation of house management, routines, programs and activities. Promote skill development, provide opportunities for choice and decision making, enhance community participation and build relationships. Write a monthly progress note to submit to the Program Manager. Assist with and develop daily living skills, to include morning and/or evening routine, personal hygiene, housekeeping, communication, nutrition, cooking and transportation. Assist individuals with scheduling appointments for medical, dental, psychological treatment, social security determination, hair appointments, etc. Assist individuals with shopping for personal items. Provide transportation arrangements for appointments and shopping, accompany, and aid as needed.Qualifications:
Previous experience with clients who have an IDD, DD, TBI, etc. is a must. Maintain a calm demeanor in the face of physical and behavioral aggression. Ability to build rapport with patients and their families. Strong problem solving and critical thinking skills. Ability to thrive in a fast-paced environment. Commitment to skill development, independent living, and full community participation for all individuals. Current medication administration and CPR/First Aid Certification required (will train). Completion of Mandt or CPI behavior training (will train). A valid NH driver's license. Auto Insurance at or above NH state minimum. Homeowners or renter's insurance. Must pass a national and local criminal background check, BEAS, DMV records check, and required TB testing. Compensation for this role is based on the individual being placed.Salary:
