Job Description
Job Overview We are seeking a compassionate and dedicated Home Support Worker to provide exceptional care and assistance to individuals in their homes and community settings. This vital role involves supporting clients with daily activities, health monitoring, and emotional well-being, ensuring they live safely, comfortably, and with dignity. The ideal candidate will be energetic, patient-focused, and committed to enhancing quality of life through personalized care. This paid position offers an opportunity to make a meaningful difference in the lives of those needing support with aging, disabilities, or chronic health conditions. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe for any changes in health or behavior Remind to take medication as prescribed and maintain accurate care plans in compliance with HIPAA regulations Prepare nutritious meals tailored to dietary needs and assist with food intake when necessary Operate assistive devices such as Hoyer lifts and perform heavy lifting tasks safely when required Support clients during outings or community activities to promote social engagement and independence Maintain a clean and safe environment through regular cleaning tasks and infection control practices Document patient progress, update care plans, and communicate effectively with healthcare teams Skills Proven experience in home health, assisted living, or long-term care environments Knowledge of patient care techniques including Alzheimer's, dementia, memory care, autism spectrum disorder (ASD), and developmental disabilities Certification in CPR and First Aid; experience with hospice or palliative care is a plus Ability to work with individuals requiring specialized support such as Alzheimer's or dementia care Strong observational skills for patient monitoring and reporting changes in condition Familiarity with HIPAA regulations to ensure confidentiality of patient information Experience using mobility aids like Hoyer lifts and performing tasks involving heavy lifting safely Excellent communication skills for working collaboratively with clients, families, and healthcare professionals Join us as a Home Support Worker if you're passionate about providing compassionate care that empowers individuals to live their best lives.
