Job Description

Outreach Coordinator - Caregiver Support Western Queens The Outreach Coordinator leads strategic outreach efforts to build awareness of program services and expand access for family caregivers. This role is responsible for developing and sustaining partnerships with community-based organizations, healthcare providers, social service agencies, and other key stakeholders to strengthen referral pathways and increase program visibility and caregiver enrollment. The ideal candidate brings strong communication and organizational skills, along with demonstrated experience in planning, relationship-building and community engagement. They will proactively identify outreach opportunities, represent the program in the community, and translate program offerings into clear, accessible messaging that resonates with diverse audiences. Through these efforts, the Outreach Coordinator plays a critical role in connecting caregivers and older adults to essential services and ensuring equitable access across the community. The Sunnyside Community Services (SCS) Caregiver Support Western Queens Program provides information, referrals, counseling and respite to family caregivers - those family members or friends who are providing care to physically disabled or cognitively impaired older adults, grandparents/kinship caregivers raising children, and older adults caring for a disabled adult. Respite service provides temporary relief from caregiving responsibilities on a limited basis and can include in-home care or adult day care services. Responsibilities Plan, coordinate, and conduct outreach events, presentations, and training across Queens community districts 1-7 which includes Astoria, Jackson Heights, Flushing, Whitestone, Forest Hills, and Ridgewood among others. Promote caregiver program services through in-person outreach and remote engagement (email, phone, and virtual platforms). Collaborate with other agency programs to schedule presentations to professionals and community stakeholders. Develop and deliver presentations and workshops for caregivers on topics such as caregiving, aging, health and nutrition, and self-care. Respond to program inquiries and support the Director with follow-up and engagement. Coordinate and implement outreach and marketing activities to increase program visibility. Track and report outreach activities, event participation, and client engagement in program databases. Use a tablet to enter client profile information on-site and track unduplicated client participation goals Perform other duties as assigned. Qualifications Associate degree in a related field, or high school diploma/equivalency with at least two years of relevant outreach or community engagement experience. Bilingual Spanish/English required. Experience conducting community outreach, participating in street outreach events, and developing community partnerships and resources. Strong interpersonal, public speaking, and written communication skills, with the ability to engage diverse community members, clients, and partner organizations. Strong computer skills, including managing virtual presentations and maintaining accurate documentation, tracking, and reporting in Excel, Salesforce, and VIVE. Strong organizational and time management skills with the ability to coordinate multiple responsibilities and across multiple databases. Ability to work independently and collaboratively in both home and office settings. Ability to maintain client and agency confidentiality. Physical Requirements and Working Environment Travel by all forms of public transportation or car in the community; walk upstairs if there is no elevator. Office work will be primarily at the SCS location in Long Island City, with field work for outreach, and telecommuting when applicable. SCS currently operates on a hybrid model with three days in the office, and two days remote. This is subject to change. Some weekend and evening hours.
