Duties Provide personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient health by taking vital signs, observing for changes, and reporting findings accurately while adhering to HIPAA privacy regulations. Assist with medication administration and ensure care plans are followed precisely to meet each client's unique needs. Support clients with meal preparation, feeding, and nutritional needs while maintaining a clean and safe environment. Operate equipment such as Hoyer lifts and manage patient transfers safely to prevent injury. Conduct regular patient observation to identify any health concerns or behavioral changes, especially in residents with dementia or autism. Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with healthcare standards. Perform light housekeeping tasks including cleaning to ensure a hygienic living space that promotes health and comfort. Collaborate with healthcare professionals and family members to develop and update individualized care plans. Provide emotional support and companionship to foster a positive environment for clients in their homes or community settings. Requirements Prior experience in home health, assisted living, group homes, nursing homes or long-term care environments preferred. Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus. Demonstrated ability to work with individuals with developmental disabilities, Alzheimer's/dementia, autism or other special needs. Strong observational skills for patient monitoring and recognizing urgent health issues promptly. Physical ability to perform heavy lifting tasks safely and assist with mobility using equipment like Hoyer lifts. Knowledge of medication administration protocols and adherence to safety standards in healthcare settings. Excellent communication skills for effective collaboration with clients' families and healthcare teams.
