Job Description
Responsibilities:
The Hospice Aide is a paraprofessional member of the hospice team. Major responsibilities include the performance of personal care activities and related services in which he/she has been evaluated as competent, thus ensuring quality and safe delivery of home health aide services.Essential Functions:
- Performs personal care activities and related services in a professional and courteous manner under the supervision of a Registered Nurse or Care Delivery Administrator (RN).
- Notifies supervisor of abnormalities or changes in the patient's condition in a timely manner and documents findings.
- Promotes and maintains infection control procedures and utilizes universal precautions.
- Participates with implementation and revision of the patient's care plan as established by a registered nurse and in accordance with agency policies and procedures.
- Uses equipment and supplies according to the HHA care plan, manufacturer's instructions, and other appropriate guidelines in order to ensure safety at all times.
- Assists patients with transfers, ambulation and exercises as assigned.
- Demonstrates effective communication skills with patients, families, caregivers, and other team members.
- Carries out all assignments as requested. Completes and submits all necessary documentation daily according to policy and procedure.
- Demonstrates knowledge of patients' rights and responsibilities.
- Participates in new employee orientation, case conferences, staff meetings, chart audits and in-services as requested.
- Performs other supportive tasks as requested.
Qualifications:
- High school graduate or equivalent.
- Certification as a Certified Nursing Assistant in the State of South Carolina.
- Demonstrated ability to carry out written and oral instructions.
- One year clinical experience preferred.
- Reliable transportation with liability insurance, current driver's license, and a good driving record.
- CPR Certified.
- Current PPD and/or chest x-ray.
- Excellent patient relationship skills
- Ability to work independently with minimal supervision.
- Ability to work under pressure with time constraints.
- Ability to adapt to variable home environments and hygiene standards.
