What You Will Do:
Follow individualized plans of care (ISP) for each assigned resident Observe residents for changes in condition and report concerns promptly to the nursing supervisor Document daily care and any notable changes throughout the shift, especially during overnight rounds Check and record vital signs, weights, and perform other duties as assigned in accordance with state regulations and community policies Assist residents with activities of daily living (ADLs), including bathing, dressing, toileting, and oral care Provide restorative nursing support, including positioning, ADL training, and passive range of motion Offer cues, redirection, and emotional support to residents as needed Encourage and assist residents in participating in activities and programs Assist with transfers and ambulation using proper techniques and equipment Respond promptly to call lights and urgent needs Provide transportation within the community and occasionally accompany residents to outside appointments Support activities and other departments as needed Perform other duties as assigned
Why You Are Qualified:
High school diploma or equivalent (GED) required Completion of a State-approved Nursing Assistant Training Program (if required) CNA certification preferred CPR and First Aid certification (if required) Ability to perform physical tasks including standing, walking, bending, lifting, and assisting residents throughout the shift Must be able to lift up to 50 pounds and assist with resident transfers using proper equipment
