The Village at Deane Hill POSITION SUMMARY
The primary responsibility of the Care Manager is to assist residents who are functionally, physically, and/or socially impaired and require assistance with activities of daily living. Under the supervision of the Director of Nursing and/or Assistant Director of Nursing, the Care Manager promotes resident dignity, privacy, independence, freedom of choice, safety, and quality of life. This is a direct-care position. In addition to resident care responsibilities, the Care Manager supports resident dining services and food safety operations. QUALIFICATIONS REQUIRED
High School Diploma or GED preferred. CNA, HHA, PCA
certification or equivalent caregiving experience preferred. One year of caregiving experience preferred. Food Handler Permit required or obtained within the timeframe required by state regulations. Ability to communicate effectively with residents, families, visitors, and team members. Compassion for and desire to work with older adults. RESPONSIBILITIES
Assist residents with activities of daily living including bathing, dressing, grooming, toileting, mobility, transfers, positioning, and continence care. Promote resident independence, dignity, privacy, and choice. Observe and immediately report changes in resident condition, behavior, or care needs. Maintain accurate and timely documentation of care provided. Assist with resident personal laundry and community linens as assigned. Encourage resident participation in activities and social opportunities. Greet and assist visitors in a professional and welcoming manner. Assist in maintaining cleanliness of resident rooms, dining areas, and common areas. Follow resident service plans and community policies. Attend required trainings, drills, and in-service programs. Report incidents, accidents, hazards, and unsafe conditions immediately. Perform other duties as assigned. RESIDENT DINING SERVICES
Prepare dining areas for meal service. Plate and serve resident meals in accordance with dietary requirements and community standards. Assist residents during meal service and provide dining assistance as needed. Escort residents to and from dining areas as appropriate. Monitor food and fluid intake and report concerns. Ensure residents receive meals consistent with dietary restrictions, allergies, preferences, and physician-ordered diets. Create a positive dining experience through courteous and attentive service. Clear tables, remove dishes, and maintain dining room cleanliness. Assist with special meals, celebrations, and community events. FOOD SAFETY AND KITCHEN MANAGEMENT
Ensure all food stored in resident service kitchens, nourishment stations, refrigerators, freezers, and pantries is properly labeled, dated, and current. Conduct routine inspections of food storage areas to ensure products are not expired and are safe for consumption. Remove and appropriately dispose of expired, unlabeled, or improperly stored food items. Maintain cleanliness, organization, and sanitation of kitchens, nourishment stations, and food storage areas. Follow all food safety, sanitation, and infection-control procedures. RESIDENT RIGHTS, CONFIDENTIALITY, AND SAFETY
Treat all residents with dignity, respect, compassion, and professionalism. Maintain strict confidentiality of resident, employee, and community information. Support resident rights, privacy, and person-centered care. Immediately report suspected abuse, neglect, exploitation, mistreatment, or violations of resident rights. Cooperate fully with all investigations and reporting requirements. DEMENTIA CARE AND RESIDENT ENGAGEMENT
Utilize dementia-sensitive communication techniques when interacting with residents experiencing cognitive impairment. Respond appropriately to behaviors associated with dementia while maintaining resident dignity and safety. Foster meaningful resident interactions and relationship building. Support resident emotional well-being and participation in community life. LEADERSHIP EXPECTATIONS
Complete duties with minimal supervision. Demonstrate professionalism, accountability, teamwork, and sound judgment. Maintain flexibility to meet resident and community needs. Follow all Willow Ridge Senior Living policies and procedures. PHYSICAL / SENSORY REQUIREMENTS
Ability to exert 20-50 pounds of force occasionally and/or 10-25 pounds frequently and/or up to 10 pounds constantly to move, lift, carry, push, pull, or transfer residents and equipment. Ability to stand, walk, sit, bend, stoop, kneel, crouch, reach, twist, and perform movements associated with resident care and dining service activities. Must be in compliance with all required vaccinations and health requirements. OSHA CLASSIFICATION
