Job Description

Are you a compassionate, dependable individual who wants to make a real difference in someone's life? We are seeking caring and committed Family Model Providers to open their homes to adults with intellectual and developmental disabilities (IDD) through Tennessee's DDA Family Model Program. As a Family Model Provider, you will welcome an individual into your home and support them as a true member of your household — not as a client in a facility. This is a live-in role that offers meaningful, rewarding work alongside a flexible, home-based lifestyle. What You Will Do  Provide a safe, clean, and structured home environment for the individual in your care.  Support daily living activities including hygiene, meal preparation, and household routines.  Administer medication reminders and monitor the individual's health and well-being.  Transport the individual to medical appointments, community activities, and day programs.  Implement the individual's personalized Individual Support Plan (ISP) in collaboration with the agency.  Teach and reinforce life skills such as cooking, budgeting, and personal care to promote independence.  Encourage community integration through social outings, faith-based activities, and employment programs.  Maintain accurate daily documentation and communicate regularly with agency coordinators.  Act as a consistent, positive, and nurturing presence in the individual's life. Requirements Residence Must own or rent a safe, accessible private home with a dedicated bedroom available for the individual. Background Check Must pass criminal history, abuse registry, and driving record checks.
Driver's License:
Valid Tennessee driver's license required.
Transportation:
