Job Description

Shifts available: 2pm-10pm, 6am-2pm, and 10pm-6am
Description of Position:
Provide overall care and support to the residents of Maple Cottage Assisted Living. Promote resident wellbeing and satisfaction through direct personal care and support with activities of daily living. Complete housekeeping assignments and follow the lead and coaching of the Lead Care Partners. The nurse on shift is always the charge nurse. Care Partners will follow directions given by the nurse on shift.
Staffing Pattern:
Maple Cottage Assisted Living staffs through a primary care structure. Each Care Partner will be charged with all, of the personal care duties for each resident assigned. Whenever possible each Care Partner will be assigned the same resident group each day to promote continuity of care. Responsibilities of the
Care Partner:
Assist with activities of daily living, including assistance with resident self-medication. Follow licensing regulations, safety policies and direction at Maple Cottage, including proper lifting technique and universal precautions when providing care to the residents. Be knowledgeable of the resident's care plan and refer to it in the performance of personal care duties. Monitor resident activity, food intake, functional status, and psychosocial status. Take action, as required, to promote resident wellbeing. Report status changes and follow direction given by the nurse on shift. Act immediately on any resident crisis, following protocol and policy. Promote open communication between health care professionals, families, residents and staff. Complete Care Partner assignments including but not limited to, cleaning and laundry, making rounds, dining services, activities, and tasks as directed by supervision. Function as a team, assisting coworkers as the need arises and following Lead Care Partners. Adhere to guidelines in the employee handbook. Complete training assignments Other duties as assigned
