Provide hands‑on care: assist clients with personal hygiene, mobility, light exercise, meals, housekeeping, errands, and medication reminders. Ensure quality & safety: complete home‑safety assessments, conduct routine quality‑assurance visits, and document plan‑of‑care updates for the Care Manager. Train & onboard caregivers: deliver one‑on‑one coaching, skills refreshers, and ongoing competency checks. Serve on the on‑call rotation: triage caregiver call‑outs, fill urgent shifts, and resolve client concerns after hours. Problem‑solve in real time: address changes in client condition, escalate issues, and keep families informed. What You'll Bring 1+ year of professional CNA, or home‑health experience (lead/supervisory duties a plus) First Aid & CPR certification (or willingness to obtain—paid by employer) Proven ability to coach peers and communicate effectively with clients, families, and clinical staff Reliable transportation, valid driver's license, and ability to lift/transfer clients as needed Tech‑savvy enough to document care notes in a mobile app or EMR Compassionate, ethical, and calm under pressure—professional in every scenario Ready to Lead with Heart?
