Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, including vital signs and overall well-being Medication Reminders Prepare nutritious meals tailored to individual dietary needs and preferences Support clients with mobility using equipment such as Hoyer lifts or transfer belts, ensuring safe handling during heavy lifting or transfers Maintain accurate documentation of care provided, patient observations, and any changes in condition Perform light housekeeping duties like cleaning, laundry, and maintaining a safe living environment to promote hygiene and comfort Requirements Proven experience in home & community care or assisted living environments; group home experience is a plus Knowledge of patient observation techniques, medication administration, and vital signs monitoring Experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer's), or memory care needs Ability to lift heavy objects safely and assist with patient transfers using proper equipment like Hoyer lifts or resident's Strong communication skills to collaborate effectively with clients, families, and healthcare professionals Compassionate attitude with a commitment to providing respectful, personalized care in accordance with care plansThis role is paid and offers an engaging opportunity for dedicated caregivers eager to support individuals in their homes while developing valuable skills.
