Participates in the development and implementation of client plans of care per home care agency policy and procedure, as appropriate. SUPERVISOR

in a timely manner. Service plans of care are discussed with the Supervisor on a regular basis. Client assignments and reports are received from the Supervisor.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

• High school graduation or GED required.
• Shall have one year of full-time experience in personal care or an institutional setting, such as a hospital or nursing home, OR shall have one year of full-time experience within the last five years in direct client care in a home care agency setting.
• Demonstrated ability to read, write, and carry out directions.
• Evidence of maturity and ability to deal effectively with job demands
• Good verbal and written communication skills required.
• Attends (8) hours of personal care oriented in services per year.
• Participates in professional meetings when directed.
• Shall have a criminal history check conducted before being offered work with this agency.
• Ability to work closely supervised to ensure competence in providing client care.

PHYSICAL DEMANDS

• Must be able to read & write English.
• Must be physically able to lift, bending, and standing for long periods.
• Must be able to handle stressful situations due to clients' medical conditions.
• Must adhere to assigned and agreed work schedule.
• Able to drive up to 50 + miles per day; Must have driver's license.
• Must maintain a professional appearance and good personal hygiene

Pay:

$15

• 17/per hour DOE Annual salary range based on 20 PT hours: $15,600 to $17,680 Background requirements: Candidates must pass a drug screen, background check and have a clear driving record.
