Responsibilities:

Provide one-on-one care and support to clients in their homes, including assistance with daily living activities, medication reminders, and personal care Monitor clients' conditions and report any changes or concerns to the on-call team and/or healthcare providers Engage clients in activities that promote physical and emotional well-being, such as reading, puzzles, or games Maintain accurate records of client care and activities Collaborate with other healthcare professionals to develop and implement personalized care plans Demonstrate a commitment to the SYNERGY Effect, providing care that not only meets but exceeds client expectations About the

Requirements:

High School Diploma 6+ months experience in home care or a related field Ability to lift 25 pounds and stand for extended periods Valid driver's license and reliable and insured transportation Ability to work independently and as part of a team How to
