Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient health by checking vital signs, observing changes in condition, and reporting findings accurately. Prepare nutritious meals tailored to dietary requirements and assist with meal planning and cooking. Administer medications safely following care plans and HIPAA regulations to protect patient privacy. Support clients with mobility using equipment such as Hoyer lifts or wheelchairs, ensuring safety during transfers. Maintain a clean, safe environment by performing light housekeeping tasks like cleaning and laundry. Document care activities meticulously, update care plans regularly, and communicate effectively with healthcare teams. Qualifications Proven experience in home health, assisted living, nursing homes, or group homes—preferably with elderly or disabled populations. Knowledge of patient observation techniques, medication administration, and vital signs monitoring. Certifications such as CPR, First Aid, HIPAA compliance training are highly desirable. Experience working with individuals with developmental disabilities, autism spectrum disorder (ASD), dementia or Alzheimer's disease is a plus. Ability to lift heavy objects safely and perform physically demanding tasks as needed. Strong communication skills to collaborate effectively with clients, families, and healthcare professionals.
