This client requires hands-on care, close supervision, and a patient caregiver who is experienced with dementia support and personal care assistance. Client Care Needs Late-stage dementia care and memory support Continuous supervision due to high fall risk Assistance with sit-to-stand transfers using a gait belt Full assistance with bathing, dressing, and incontinent care Oral hygiene setup assistance Redirection and reapproach techniques when care is initially refused Meal assistance and companionship during meals Ensure client safety and comfort throughout shift Schedule Monday, Wednesday, and Friday 3:00 PM
