Job Description

Job Summary Join our dedicated team as a Home Provider, where you will play a vital role in delivering compassionate, personalized care to individuals with diverse needs. As a Home Provider, you will be responsible for assisting residents with daily activities, monitoring health and safety, and fostering a warm, engaging atmosphere. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor residents' health by taking vital signs and observing for any changes in condition or behavior Prepare nutritious meals tailored to individual dietary needs and preferences Maintain accurate care plans and document all patient interactions, observations, and interventions in compliance with HIPAA regulations Administer medications safely following prescribed instructions and assist with medication management as needed Support residents during medical appointments or outings within the community Employ manual handling techniques such as the use of Hoyer lifts to ensure safe transfers and mobility support Engage residents in social activities and provide companionship to foster mental stimulation and emotional well-being Maintain a clean, safe living environment through regular cleaning and organization of resident spaces Follow all safety protocols including CPR and First Aid procedures to respond effectively during emergencies Collaborate with healthcare professionals, social workers, and family members to develop comprehensive care plans tailored to each individual's needs Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health settings with individuals who have developmental disabilities or neurodevelopmental disorders Strong patient observation skills to detect changes in condition or behavior promptly Knowledge of dementia care, Alzheimer's care, memory care, and hospice support strategies Ability to perform manual handling tasks such as heavy lifting and the use of assistive devices like Hoyer lifts safely Familiarity with medication administration protocols and HIPAA compliance standards Excellent caregiving skills with a focus on patient-centered support for seniors or individuals with disabilities Effective communication skills for working collaboratively with team members, residents, families, and healthcare providers Ability to assist with ADLs efficiently while maintaining dignity and respect for residents' independence Basic knowledge of vital signs monitoring and health assessment techniques Experience in meal preparation that adheres to special dietary requirements Compassionate attitude combined with patience, adaptability, and strong organizational skills Join us in creating a nurturing environment where every individual receives respectful, attentive care.
