Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by checking vital signs and observing for any changes in condition Prepare nutritious meals tailored to individual dietary needs and preferences Administer medications accurately following care plans and HIPAA privacy regulations Operate mobility aids such as Hoyer lifts and assist with transfers safely and comfortably Maintain detailed documentation of care provided, patient observations, and any incidents or changes in condition Support clients with cognitive impairments such as dementia or Alzheimer's through memory care techniques and engagement activities Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment Collaborate with healthcare professionals to develop and implement personalized care plans Provide emotional support and companionship to foster a positive environment for clients Qualifications Prior experience in home health, assisted living, nursing homes, or group homes is highly desirable Certification in CPR and First Aid required; additional certifications such as HIPAA training preferred Strong patient observation skills with the ability to recognize changes in health status promptly Experience working with individuals with developmental disabilities, autism, or dementia is advantageous Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary Knowledge of medication administration protocols and vital signs monitoring Compassionate attitude with excellent communication skills and a respectful approach to client independence Valid driver's license and reliable transportation preferred for travel between client locations Join us in delivering exceptional home care that transforms lives!
