Job Description
Job Overview We are seeking a compassionate and dedicated Caregiver to provide exceptional home-based care for clients in League City. This vital role involves supporting individuals with diverse needs, including seniors, those with developmental disabilities, and individuals requiring specialized memory or dementia care. The ideal candidate will bring energy, empathy, and a proactive approach to enhancing our clients' quality of life. You will be responsible for delivering personalized assistance, ensuring safety, and maintaining a warm, respectful environment that promotes independence and well-being. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and observe patients' health status, including vital signs and overall condition, documenting changes accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications safely following medical documentation protocols while respecting HIPAA regulations Support clients with meal preparation, cooking, laundry, cleaning, and other household tasks to maintain a clean and comfortable living environment Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral management techniques Assist with medical procedures such as catheter care, wound care, and hospice support as needed Maintain thorough medical records and documentation of patient care activities in compliance with healthcare standards Use medical terminology accurately when recording observations or communicating with healthcare professionals Employ patient observation skills to identify changes in condition promptly and escalate concerns appropriately Work effectively within assisted living or home & community care settings to promote independence and safety Qualifications Proven experience in senior care, home health, or long-term care environments; nursing home experience is highly valued Knowledge of medical terminology, medication administration, and basic nursing skills such as vital signs measurement and catheter care Certifications in CPR (Cardiopulmonary Resuscitation) are required; additional certifications like hospice care or dementia training are a plus Experience working with individuals with developmental disabilities or memory impairments such as Alzheimer's disease is preferred Familiarity with EMR/EHR systems for accurate documentation of patient information Ability to perform physical tasks including lifting using Hoyer lifts and assisting with mobility needs Strong communication skills to collaborate effectively with clients' families and healthcare teams Compassionate attitude combined with the energy to provide engaging and respectful care Join us in making a meaningful difference in the lives of those we serve!
