Department. We are an agency focused on delivering compassionate, high-quality care to patients and clients in their homes or places of residence, including non-medical personal assistance, skilled nursing, and rehabilitation. Our caregivers provide high quality and timely care, as we focus on each unique patient to deliver the appropriate care and support to our patients and their families. As a member of the home care team, the Caregiver works under the supervision Primary Home Care (PHC) Supervisor and performs various personal care services and assist with activities of daily living as necessary to meet the client's needs. The personal support worker is responsible for providing personal tasks as stipulated on the care plan. The Caregiver will be assigned in a manner that promotes quality, continuity and safety of a client's care. This position must be able to drive to Grand Prairie , Texas.

Position Requirements :

Must be available from 5:00 pm- 9:00 pm

Job Summary:

As directed by the PHC supervisor, provide client care needs Primary function is to provide personal assistance services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. As a Personal Attendant, you will actively engage clients in a meaningful way to add value to their lives. You will assist clients with the following: Activities of Daily Living, Personal Care Maintain an orderly client environment Follow principles of infection control and Universal Precautions Identifies and participates in performance improvement activities Assist in providing a safe environment and promote quality patient care through adherence to established policies, procedures and standards Ensure client's rights are adhered to. Use equipment properly and in a safe manner Respond rapidly and appropriately in an emergency situation Responsible for punching /out from client's home when delivering services Must have reliable transportation to client's home

Qualifications:

Minimum of 6 months to 1 year of documented experience required. Licensed driver with automobile that is insured in accordance with the organization's requirements. Self-directing with the ability to work with little direct supervision Empathy for the needs of the client Demonstrates interest in the welfare of ill and elderly. The candidate of choice will have the following background and qualifications: Competent to perform tasks assigned by the supervisor. Certified Nursing Assistant preferred but not a must. Treats clients, staff and the public with courtesy, respect and presents a positive public image. Ensures confidentiality and security of the client's medical information.
